Health and social work services. The Royal Borough of Kensington and Chelsea, in partnership with Westminster City Council, The London Borough of Hammersmith and Fulham and Central London Clinical Commissioning Group are working together to build local volunteer led services for older people. It is hoped the services will help older people enjoy more choice and control in their lives and feel the benefit of having someone around when they need them most. The Royal Borough of Kensington and Chelsea will be leading the procurement which is packaged into the following contracts.
Short Description of Services.
Lot 1 - Hospital to Home Service: Assisted discharge service for residents of the Tri-borough councils from local hospital sites to home, immediately after a hospital episode. This volunteer led service will help facilitate the discharge process in the local hospitals and will aim to divert people to the community whilst avoiding further hospital admissions. The approach will provide effective support to the hospital and care management teams.
Lot 2 - Befriending Plus Service (Westminster Only): Befriending for older isolated people registered to a Central London Clinical Commissioning Group Practice. This volunteer led service aims to serve as low level preventative intervention and improve the quality of individual lives, including physical and mental health well being.
We are seeking Expressions of Interest from suitably qualified organisations interested in tendering for the provision of the services set out in the Lots listed above. Organisations may express an interest in one or either or both Lots. In order to make an Expression of Interest a Pre-Qualification Questionnaire (‘PQQ’) must be completed and returned. PQQs may be obtained by email request to charles.stephens@rbkc.gov.uk and must reference the Lot or Lots the organisation is interested in. Completed PQQs must be returned by 1pm on Friday 21.12.2012.
The Tri-borough councils are seeking a contractor for the provision of a Hospital to Home service and/or a befriending service (Westminster only). Applicants should have some local knowledge and be able to demonstrable experience of working with volunteers. The service will be based on the current model which has been tested.
There will also be the opportunity for the successful tenderer of either Lot 1 or Lot 2 (or both) during the first contract year to develop a hospital discharge link project to improve the experience of hospital discharge amongst older people.
Maximum Budget Allocations.
Lot 1: £666 084.
Lot 2: £100 000.
The budget allocation for Lot 1 is based on a two year contract with the option to extend for, up to a maximum of, 2 further years and includes the cost of start-up.
The budget allocation for Lot 2 is based on a two year contract.
