County Durham and Darlington Foundation Trust are seeking to commission a number of nursing home beds in the Durham City/Bishop Auckland area as a short stay ‘step down’ facility for patients following an in-patient stay within the University Hospital of North Durham and Bishop Auckland General Hospital. The beds are required for patients who are medically fit for transfer/discharge out of hospital setting but who are not able to return home with support and an admission to the nursing home is the most appropriate place for the patient to receive the required input for a maximum of 6 weeks, but may be for 1 week or less Model - gatekeeping, goal setting and discharge planning of these patients will be the responsibility of RIACT North ( Responsive Integrated Assessment and Care team) who are a team of health and social care staff responsible for who deliver rehabilitation, recovery and re-ablement services via a seamless pathway of care, ensuring that duplication and multiple assessments are minimised and levels of physical functioning are maximised. Criteria - Nursing Home providers within 2 mile radius of UHND and BAGH with a minimum of 6 available beds, to be utilised between Dec. 2012 and end March 2013
