Barnet Council is seeking to commission three practical support services to support the prevention and personalisation agenda in Adult Social Care and Health.
The Council wishes to invite expressions of interest from organisations and consortia able to provide a modern, inclusive and innovative package of older people's support services to complement its reconfiguration of day opportunities services for older people.
Practical support services will contribute to the achievement of Adult Social Care and Health's strategic priorities of driving forward improvements in the health and wellbeing of our population by ‘Keeping Well, Keeping Independent'. It is intended that these services will provide timely interventions at the right moment and therefore help to enable people to continue to live independently at home.
Service delivery will be underpinned by key principles of:
— Enabling people to return home safely and remain safely at home,
— Prevention of hospital admissions or admissions to permanent care,
— Enabling timely consideration of and planning in response to the factors which contribute to an active and healthy later life,
— Promoting independence and positive experience of services,
— Increasing numbers of trained volunteers,
— Improving partnership working and integration across healthcare services.
We are particularly interested to hear if providers have new and innovative ways of delivering these types of support.
Contracts are to be awarded from May 2013 for 3 years with an opportunity to extend the period for a further 2 years, subject to review.
The tender opportunity comprises of two seperate Lots:
— Lot 1: Home from Hospital and Handyperson,
— Lot 2: Later Life Planning.
Organisations are invited to bid for one or both lots.

Contract value: Suppliers must provide responses to the Questionnaire in Tender Box AG4GB3W494 before completing the Lot specific Method Statement questions. Failure to complete the Questionnaire in its entirety will result in bidders being removed from the process and their Lot specific responses not being taken further.Scores from the Questionnaire will be combined with the scores from the Lot specific Method Statements to calculate the overall score for each Lot.Estimated value excluding VAT: Range: between 620 000 and 660 000 GBP
