Provision of Any Qualified Provider (AQP) for Continuing Care Packages for Children and Young People across Nottinghamshire

NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU) on behalf of Nottingham and Nottinghamshire Clinical Commissioning Partnership is inviting suitably qualified providers to deliver Continuing Care Packages for Children and Young People (0-24 years) across Nottinghamshire under an Any Qualified Provider (AQP) process, 2nd window. Services are required for tailor made package of care needed over an extended period of time for children and young people with complex health needs which arise because of disability, accident or illness (including life limiting/ life threatening conditions or mental health illness) that cannot be met by existing local universal and specialist healthcare services alone. The procurements main aims are: - To extend to the current list of approved providers, who can deliver high quality, outcome focused packages of care for children and young people living at home with their families. - To develop a service which can meet the highly complex health needs of children and young people in Nottinghamshire in accordance with the principles of the National Framework for Children and Young People with Continuing Care guidance and the national personalisation agenda. This AQP does not guarantee any volume of activity or payment to a provider. For the avoidance of doubt all AQP contracts will be let on a zero value, zero volume basis.
